*Remember, if you were granted "Retired-Inactive" status in 2012, you do not need to fill out a form.

To fill out the electronic form, you need to have Adobe Reader. If you wish to save your data in the form as a .pdf for future viewing, you will need to have Adobe Acrobat, a paid component, installed. Otherwise, be sure to print all the copies you will need before closing the form (1 for your records, 1 for CCIW, and other as needed). Follow these steps

  1. download the form
  2. save it to your computer
  3. fill out the form
  4. sign the form electronically (click the electronic signature box and follow the on-screen instructions).
  5. print copies

Make sure you save the form often during and after completion when working in Adobe Acrobat.

When using the electonic signature, please do not worry if you receive an error regarding the validity of the signature. Please just make sure your signature appears in the signature box and your other information shows in the other fields.

You may send electronic forms by email by using the "attachment" feature in your mail program.

Please be sure to fill out your form completely and type or print legibily in ink. Please return your form postmarked no later than January 23rd, 2013 via one of these methods:
